Show item references when the agenda is published.

Description

Now, item references are shown only when decisions are temporarily published. We should show them earlier, when the agenda is first published.

Done. The item references are shown as soon as the meetings are published, and updated accordingly every time an item is added or removed from a published or created meeting. When a meeting is closed, normally no more items can be added to it, so firstItemNumber for the meeting is hardcoded at this moment.
